If you have dental insurance please call the office and we will give you instructions as to what to bring in.

Patient Forms

To decrease wait time in our office, we recommend you download and print our new patient forms. When you come to your first appointment, bring in these documents and make sure all the necessary information has been filled out and signed. This will make your appointment session much quicker and you won’t feel rushed during the on-boarding process.

Our office will gladly answer any questions you may have on our payment options, as well as estimate your insurance deductible. Should you wish to seek more extensive dental treatments, our office also offers financing through CareCredit®.  

In order to assist you with your health care investment, we are providing the following payment options:

• Cash — includes money orders and personal checks.
• Visa/Master Card/Discover— we accept credit cards as a payment for treatment to the extent of your credit limit.
• CareCredit® — offers a separate line of credit and approval typically takes less than 10 minutes.
